Meet Our Cancer Program Facilitator

Kathy KlebertA facilitator (or navigator) is an advocate for patients and their caregivers by coordinating services and guiding patients through the health care system.
 
At HSHS St. Elizabeth’s Hospital, our cancer program facilitator is a part of your health care team. She acts as liaison for all patients by identifying and connecting each patient with the resources that they need. She will help guide you through the process of learning about your health and offer support every step of the way.

You can feel confident that our team is committed to understanding your individual needs and assisting you during your specific course of treatment. We at St. Elizabeth’s Hospital want you to know that you are not alone on your health care journey.

Your Cancer Program Facilitator will:
  • Guide you through the health care system and help coordinate care with all members of your health care team
  • Help you keep track of appointments and find transportation
  • Provide education about your disease to you and your family
  • Identify & connect you with resources
  • Guide & empower you as you make decisions
  • Provide support & advocate for you during treatment

This role supports hospital inpatients meeting set criteria and receives referrals from physicians for outpatient studies and cancer support. For more information, call 618-624-3668.
