ChartMyHealth Online Health Record Now Available


ChartMyHealth is a free, online personal health record that can be securely accessed from a home computer, laptop, mobile device or smartphone. It is one of the newest and easiest tools for health care management.

ChartMyHealth gives patients the ability to see a portion of their medical record, such as laboratory results, current medication list and medical allergy list.


Patients can sign up for ChartMyHealth during the registration process at St. Elizabeth’s Hospital. They can also visit Health Information Services (Medical Records) on the main floor of the hospital anytime from 8:00 a.m. to 4:30 p.m., Monday through Friday to sign up. All participants must have an email address and provide one form of identification.

Please visit the Health Information Department to sign up a child. A copy of his/her birth certificate or other legal proof of guardianship is required, in addition to the guardian’s photo identification.


After signing up patients will receive an email invitation from FollowMyHealth with a link to activate the account and create an authentication ID.
