St. Elizabeth’s Hospital now offers patients one of the newest and easiest tools for health care management. ChartMyHealth is a free, online personal health record that can be securely accessed from a home computer, laptop, mobile device or smartphone.
ChartMyHealth gives patients the ability to see a portion of their medical record, such as laboratory results, current medication list and medical allergy list.
How do I sign up?Patients can sign up for ChartMyHealth during the registration process. They can also visit the Health Information Services (Medical Records) Department on the main floor of the hospital anytime from 8:00 a.m. to 4:30 p.m., Monday through Friday to sign up. All participants must have an email address and provide one form of identification. Visit our ChartMyHealth page for more details.
